Job Summary:
Reporting to the Executive Director or Client Service/Branch Manager, the Home Support Supervisor proactively ensures the effective delivery of client centered services to Revera Home Health clients, while seeking opportunities to continually improve and enrich the program.

Job Requirements:


  •     RN/RPN/LPN/PT/OT/SLP/RD/SW in good standing, with appropriate provincial regulatory body ;
  •     Three (3) years experience as a professional nurse with community experience preferred;
  •     Demonstrated understanding of current health care trends and  issues;
  •     Strong leadership skills and experience managing a team;
  •     Strong problem solving ability;
  •     Proven customer service skills and ability to communicate effectively(oral & written) with both internal and external clients;
  •     Ability to multitask and adapt to fast paced environment;
  •     Experience with Microsoft Office Suite applications; (word, excel, ppt)
  •     Current (within 6 months) Vulnerable Position/Sector Screening (VPS).

Job Duties:

  •     To assess client care needs, initiate client visits and the development of an appropriate plan of care that identifies care objectives and anticipated outcomes as applicable
  •     To  conduct follow-up visits/calls with clients to ensure the service provided meets the client's needs and adheres to RHH quality standards
  •     To participate in the implementation of administrative policies and procedures relating to safe and effective client care
  •     To recruit, train, develop, supervise  & performance manage staff
  •     To plan and organize orientations and  in-service education programs
  •     To promote Revera Home Health through community involvement, committee participation
  •     To act as a clinical resource for community staff
  •     To lead regular staff meetings to engage staff and foster teamwork
  •     To address work related issues and keep staff updated on new information affecting client care
  •     To implement CQI activities, and participate in directing the collection of client care quality and outcome indicators
  •     To respond to complaints, compliments and any discrepancies, and ensure effective resolution as required
  •     To collaborate with multidisciplinary team members as appropriate: physicians, community agencies, home support providers etc.;
  •     To maintain an effective working relationship with the Executive Director, Client Service/Branch Manager and Coordinator, and relay relevant facts regarding clients and client care staff;
  •     To evaluate  care delivery  to ensure the service provided meets the client's needs,  service purchaser's contractual requirements and the company's quality standards;
  •     To consult  and participate  in Client Care Conferences as needed to ensure high quality care;
  •     To  maintain current certification for Basic Cardiac Life Support & current registration in good standing with provincial regulatory body;
  •      To establish staffing patterns/volumes  that promote  safe, quality client  care ;
  •     To ensure  that professional, high quality  standards are maintained by all  staff providing care;
  •     To promote staff development and provide opportunity for continuing education;
  •     To uphold and promote the organization's values and philosophy relating particularly to ethics, morality, and integrity as set out in Revera's Code of Conduct;
  •     To complete all other tasks as assigned.
